Personal Information Collection and Use Consent Form for 17th DMZ International Documentary Film Festival Badge Registration
To provide personalized services to individuals, the DMZ International Documentary Film Festival collects and uses the personal information and obtains consent from the applicant according to the [Personal Information Protection Act]. The collected personal information is used only for the designated purposes, and any changes in purpose of collecting the personal information will be informed in advance for the consent.
Name, email, contact information, affiliation, position, company information, supporting documents, photograph
● Identify verification and issuance for 17th DMZ International Documentary Film Festival and DMZ Docs Industry badge application
● Guest service provision during 17th DMZ International Documentary Film Festival and DMZ Docs Industry period
Personal information will be stored from August 2025 for badge registration and issuance. With your consent, we wish to retain information for 1 year for audience development and service improvement. Upon your request, this information will be destroyed after the festival ends.
You may refuse such personal information collection, but refusal may restrict badge registration.
